Instructions

Provide sufficient detail to ensure proper consideration of the nominations.

Each nomination must include a written justification (more than 100 words, but less than 300 words). The justification should address the reason the employee is eligible for the award and should be specific about the level and nature of the accomplishment.

Qualifications of a nominee might include: impact, significance, degree of commitment beyond normal job expectations, originality, etc. A separate Nomination Form should be submitted for any employee you wish to have considered for more than one award category. (Nominations for an individual for multiple awards in a single nomination form will not be accepted. If you nominated someone during a previous awards cycle and the nomination was not successful, please feel free to re-nominate.

Distinguished Career Service Award (DCSA)

A Distinguished Career Service Award may be granted annually to an FNLCR employee with at least ten years of service at the Frederick National Lab and/or NCI Bethesda campuses who has, throughout the course of his/her career, improved the quality of science and/or the work environment at the Frederick National Lab/NCI Bethesda campuses.

Please note: There are two (2) awards in this category: 1) Scientific and 2) Administrative or Infrastructure Support


How has the nominee improved the quality of science and/or the work environment? Criteria to be considered include impact and significance to the quality of science and/or the work environment at the Frederick National Lab/Bethesda campuses (more than 100 words, but less than 300 words).


Norman P. Salzman Mentoring Award (NSMA)

The Norman P. Salzman Mentoring award may be granted annually to an FNLCR scientist who most personifies Dr. Salzman's lifetime commitment to the guidance and direction of others in the pursuit of scientific excellence. Nominees for this award may be from any area of science.

Describe the nominee's mentoring activities and how they have contributed to the pursuit of scientific excellence (more than 100 words, but less than 300 words).


Outstanding Achievement Award (OAA)

An Outstanding Achievement Award may be granted annually to an FNLCR employee(s) for consistent outstanding efforts in support of stellar science at the Frederick National Lab. This award may be given on an individual or team basis. Please note that there are 4 potential awards for this category (listed below).




Describe the outstanding contribution the nominee(s) provided in support of outstanding science at FNLCR and why you feel this nominee(s) should receive this award. Criteria to be considered include impact, significance, and degree of commitment beyond job expectation (more than 100 words, but less than 300 words).


Special Achievement Award (SAA)

A Special Achievement Award may be granted to an FNLCR employee(s) for a specific achievement of a nonrecurring nature that has resulted in the support of the science performed at the Frederick National Lab and Bethesda campuses. Examples may include the completion of a special project, outstanding site visit, organization of specific research-related event, or procedure.

Please note: This award may be given on: 1) an individual basis, or 2) a team basis (all team members must be listed).


Please list the special achievement of the nominee(s) and why you feel the nominee(s) should receive this award? Criteria to be considered include impact, significance, originality and the extent beyond job expectation (more than 100 words, but less than 300 words).


The Unsung Hero Award (UHA)

The Unsung Hero Award may be granted to an individual, or group, for their contributions to the FNLCR community. This award is intended to be broad in its reach, and by design, it is meant to recognize those whose efforts have resulted in added benefit to the community and furthering the mission of the FNL.

Nominations for this award should be geared towards individual(s) whose efforts are significant and beyond the normal expectations for their role, but do not necessarily fit within the parameters of the traditional scientific achievement award definitions and criteria (more than 100 words, but less than 300 words).


Customer Relations Award (CRA)

A Customer Relations Award may be granted to an FNLCR employee who through his/her interactions with, and, services to the customer considerably enhanced the image of FNLCR. Please note that there are 2 awards for this category (listed below).


How has the nominee enhanced the image of FNLCR? Provide specific details (more than 100 words, but less than 300 words).


Cost Savings Award (CSA)

A Cost Savings Award may be granted to an FNLCR employee or employees for a suggested cost savings initiative that has been implemented and proven to have provided a significant savings to the program.


Describe the impact the cost savings initiative has had on the program. The financial impact ($) and approximate date implemented must be included (more than 100 words, but less than 300 words).


Safety Award (SA)

A Safety Award may be granted to a FNLCR employee(s) for outstanding commitment to advancing a safer work environment for the betterment of the Frederick National Lab community.

Please note: This award may be given on an individual basis and/or a team basis (all team members must be listed).


Describe the outstanding contribution the nominee(s) provided in fulfilling FNLCR's, commitment to creating a safe work environment (more than 100 words, but less than 300 words).
