Complete PR and provide the following additional information:
- Detailed description of service needed (antibody production, custom libraries, labeling
of probes, etc.)
- Describe the information or material to be transmitted to the subcontractor
- Describe the end product or test results to be received and method of receipt (e.g.
through Receiving, mail, e-mail, pickup, etc)
- Any other pertinent information, such as prior arrangements and discussions
Note: After obtaining authorization, the requestor must complete the Request for
Shipment Form.
