for Vendor Service (RVS) form is used for rotor inspections. Please indicate
the number of rotors to be inspected and the model numbers.
Fax the RVS form to the Equipment Maintenance (EM) team to initiate a service call.
All services to equipment must be initiated by the EM team. Any PR needed is created
