Helpful Hints for Completing the Frederick National Lab ASP form
This document was prepared to assist investigators with the completion of their
initial and renewal animal study proposal submissions.
Initial Proposal Submissions
Please use the Frederick National Lab Animal Study Proposal Form (Revised January 2013) for
your initial proposal submission. Be sure to reference the Frederick National Lab ACUC Guidelines
and Policies section of this website, as well as the Helpful Hints for Completing
the Frederick National Lab ASP form.
Please use the LASP Animal Numbers for Research Use Calculator if you need assistance
with preparing your animal number justification for breeding studies.
Modification to Approved Proposals
Any changes made to an approved proposal must be submitted in advance to the ACUC
for approval. Please send a memorandum detailing the modifications that you would
like to make to your approved study to the ACUC Office.
Please use the Additional Agents form if you are only adding cell lines or agents.
Any additional changes to the parent ASP (increase in animal numbers, change in
study design, dose route, dose volume, etc.) will need to be sent to the ACUC separately
as a modification.

Annual Review of Approved Proposals
Every year the PI will receive an annual review notice from the ACUC Office requesting
the PI to review the current approved proposal and to notify the ACUC of any changes.
Please be sure to sign and return this notice to the ACUC in a timely manner to
ensure compliance with Federal Regulations.
Renewal of Approved Proposals
Every three years, each approved proposal must be re-written (using the Frederick National Lab
Animal Study Proposal Form) for a PI to continue his/her research using animals.
Investigators will receive notification from the ACUC Office when their study is
scheduled to expire. Please be sure to submit your renewal in a timely manner, since
the ACUC is required to review and approve your study prior to the expiration date.

Deleterious Phenotype Reporting Form
This form is used by investigator's and/or their technical support staff to report
spontaneous and/or generated deleterious phenotypes to the Frederick National Lab Animal
Care and Use Committee.
Animal Numbers for Research Use Calculator
In an effort to assist investigators with determining the number of animals that
are necessary to support their breeding studies, LASP has designed this calculator
for your use.
Facility Space and Resource Review Form
This form is used by our Facility Managers to determine if a facility has the resources
and space to support a proposed study.
