REQUEST FOR ANIMAL SHIPMENT
INSTRUCTIONS – PLEASE READ
All requests for animal shipments from NCI-Frederick
(international or domestic) must
be placed online. There are two parts to completing a request for animal shipment.
Your request will not be processed until BOTH steps are completed:
- Submit online order form – Request for Animal
Shipment
- Fax signatory page (explained in instructions below)
Both forms must be received five (5) days prior to
the requested ship date for transfers within the United States and three (3) WEEKS
prior for international shipments.
- This application works best using Internet Explorer
5.0 or above
- Please click the TAB key between fields
- Please fill out the online order form in its entirety
– ALL FIELDS ARE MANDATORY
- After submitting your request, you will receive
an automated email (confirmation receipt) which has a link to the signatory page
- You will need to click on the link, print a copy
of the signatory page and have it signed by the Requesting PI
- Fax the signed signatory page for all
locations EXCEPT Bldg. 567 to:

After submitting BOTH the Request for Animal Shipment
form and signed signatory page, you will receive an email from the LAM office when
approval is received from the recipient. Retain copies for your records.
All questions pertaining to the Request for Animal Shipment (ordering process,
status of a request, etc.) should be directed to Sherry Stockman-Crummitt at

After submitting BOTH the Request for Animal Shipment form and signed signatory
page, you will receive an email from Rhonda Anderson at the RFAS office when approval is received from
the recipient. Retain copies for your records. All questions pertaining to the Request
for Animal Shipment (ordering process, status of a request, etc.) for Bldg. 567
